What is Medicare Case Conferencing?

Medicare Case Conferencing refers to a structured meeting where the multiple healthcare providers involved in a patient’s care come together to discuss and develop a management plan. These meetings are designed for patients with complex and chronic conditions.

Led by the patient’s General Practitioner (GP), these conferences enable allied health input. This process is a recognised and rebateable service under Medicare, reflecting the importance of coordinated care.
